How Does a Person Access Service?
- •A referral for screening can be made
directly to a hospice by a physician,
health professional, or family member
- •Eligibility determination occurs
within hours
- •Hospice is covered by most insurances
and can be provided in an assisted living
facility, long term care center, private
residence or rest home.
- •The hospice team begins working immediately
with the patient, family,
and physician.
